To: | - Departments of Health of provinces and central-affiliated cities; |
On February 5, 2022, the Government promulgated Resolution No. 14/NQ-CP on procurement of Pfizer COVID-19 vaccine for vaccination of children from 5 to under 12 years of age. In order to stay ready and achieve the maximum effectiveness when giving children within this group age COVID-19 vaccine, Ministry of Health requests:
1. Departments of Health of provinces and cities shall:
a) continue to increase COVID-19 vaccine coverage for additional and booster shots for vaccination recipients who are eligible for vaccination according to order of the Government, Prime Minister, and guidelines of Ministry of Health under Official Dispatch No. 508/BYT-DP dated January 28, 2022 and Official Dispatch No. 10722/BYT-DP dated December 17, 2021.
b) vaccinate children from 5 to under 12 years of age against COVID-19:
- Cooperate with Departments of Education and training in reviewing and producing lists of children who are studying in the first grade to the sixth grade and aged between 5 to under 12 years of age for vaccination. Cooperate with local authorities in producing separate lists for children who are in the group age but are not pursuing education.
- Develop plans and fulfill vaccination requirements in order to carry out vaccination as soon as vaccine is available.
- Type of vaccine: vaccine approved by Ministry of Health for use by the group age. Dose and interval between shots shall conform to instructions of manufacturers and National Institute of Hygiene and Epidemiology.
- Cooperate with Departments of Education and Training in organizing free vaccination campaigns at fixed and mobile vaccination sites, and schools (if in-class education is implemented). Organization of vaccination shall be implemented in accordance with Section III of Guidelines on organization of COVID-19 vaccination session attached to Decision No. 3588/QD-BYT dated July 26, 2021 and Decision No. 43/QD-BYT dated January 7, 2022 of Minister of Health.
+ Parent or guardian shall sign the Consent form attached hereto.
+ Perform pre-vaccination screening in accordance with Decision No. 2470/QD-BYT dated June 14, 2019 and Decision No. 5002/QD-BYT dated October 29, 2021 of Minister of Health.
+ Provide guidelines on monitoring and providing vaccination aftercare in accordance with Decision No. 3588/QD-BYT dated July 26, 2021 of Minister of Health.
- Publicize, mobilize, and enable the general public to promptly and adequately vaccinate children from 5 to under 12 years of age.
2. Epidemiology Institutes, Pasteur Institutes shall:
- The National Institute of Hygiene and Epidemiology shall promulgate written guidelines on COVID-19 vaccination for children from 5 to under 12 years of age and organize training for nationwide vaccination facilities.
- Epidemiology Institutes and Pasteur Institutes shall develop plans for providing training and guiding vaccination for provinces and cities if necessary, and organizing supervision and technical assistance for provinces and cities under their management during vaccination process.

CONSENT FORM FOR COVID-19 VACCINATION 1. Vaccination is an effective preventive measure, however, COVID-19 vaccine may not guarantee complete epidemic prevention. Recipients of COVID-19 vaccine are able to prevent the epidemic or reduce the severity of COVID-19 infection. After receiving COVID-19 vaccine, comply with 5K Rules for COVID-19 control. 2. COVID-19 vaccination may cause localized reactions at injection sites or systemic reactions such as inflammation, pain, headache, nausea, fever, muscle pain, etc. or severe post-vaccination complications. 3. In case of any health irregularity, parent or guardian must immediately contact the nearest medical facility for immediate examination and treatment.
